Open rhinoplasty
In open rhinoplasty, the surgeon approaches the operated area through a small incision between the nostrils in the area of the so-called columella, where an inconspicuous scar remains after the operation. Proponents of this method stress the advantage of greater clarity during surgery. On the other hand, open rhinoplasty often entails more swelling and a longer recovery.
Closed rhinoplasty
Closed rhinoplasty is performed through the inside of the nose via the nostrils without external scarring. This technique is suitable for less complex interventions in the tissues of the nose. It also usually has a shorter healing and swelling time.
Rhinoplasty with preservation of all structures
This structure preservation rhinoplasty differs from traditional rhinoplasty in that it focuses on preserving as much of the existing tissue structure as possible. The natural structures of the nose are sculpted, and there is no need to resect or remove cartilage and bone from the nose. It has the advantage of significantly faster healing and stable results. The nose is only covered with a plaster and a self-adhesive splint for 10 days. The tamponade is used to prevent bleeding in most cases, but the necessary wearing time is reduced from 2 days to only 12 hours.

Consultation
The first step to nose reshaping is a consultation with a plastic surgeon. He or she will evaluate the problem and the anatomical proportions of the nose, the condition of the tissues and the thickness of the skin that may affect the outcome of the surgery. The doctor will suggest the specific surgical procedure and type of anaesthesia, explain the possible risks, familiarise the patient with the pre- and post-operative regimen and recovery time.
The doctor will discuss with you the most suitable nose reshapingprocedure for you. Some nose reductions require intervention only in the soft tissues, while other types of rhinoplasty require intervention in the cartilaginous and bony parts of the nose as well. The rhinoplasty method you choose depends on your preference and the agreement of your doctor.
Pre-operative examination
If you decide to have the surgery at the initial consultation, you and the surgeon can agree on a date for a pre-operative examination at our clinic and the date of the surgery itself. Before the rhinoplasty, you will need to have an examination by an ENT specialist.
If the nose reshaping involves solving an ENT problem, our specialists can do this in one operation with the aesthetic treatment. This makes the whole process much gentler for you and you do not have to undergo two surgeries.
Preparing for the procedure
Talk to your doctor in advance about the medications you normally take. Some medications may need to be discontinued or replaced before the procedure. Do not rub anything on the skin in the area to be operated on the day of surgery; it should not be oily before surgery. Do not eat, drink or smoke from midnight before the procedure. Try to avoid stress and stay calm. A good mental state plays a role in recovery.
On the day of the procedure
On the day of surgery, remember to bring all the medicines you normally take to the clinic. After the procedure, which is normally performed under general anesthesia, you will stay 1-2 nights at the clinic at the discretion of the doctor. Therefore, in addition to your medications, bring a change of underwear and toiletries to the clinic. Everything else you will get from us.

Nose reshaping or reduction usually takes 45-90 minutes depending on the details and complexity of the procedure. The operation is performed under general anaesthesia through a nostril approach or through an incision in the so-called columella. In most cases, you will have a patch and splint on your nose after the rhinoplasty and a nostril tamponade by the second day after the procedure. You will then need to breathe through your mouth after the procedure.
Ice packs are applied to the nose immediately after surgery. The tamponade is removed 1-2 days after the procedure and we will let you go home from the clinic on these days. If you live in Prague or Brno, where our clinics are located, we will be happy to take you home free of charge after the procedure.
Healing time
After the tamponade is removed, you will finally be able to breathe freely, however, we do not recommend you to snifflefor the first 10 days after the removal . Clean your nose several times a day with damp cotton buds.
You will come in for a fixation check 5-7 days after surgery. You will wear this for approximately 7-10 days depending on the type of surgery. In the case of bony nose and septum surgery, you may experience swelling and bruising around the root of your nose and eyes. However, they usually heal quickly.

Aftercare
You will be on bed rest for the first few days after surgery, and you can usually return to normal activities in 2-3 weeks. The first follow-up after surgery takes place in one to 10 days, the next after one month, after 3 months and the last after ½ year. Especially during sports, you should continue to protect your nose from injury.
Enjoying the result
The healing of the tissues of the nose takes place over a long period of one year after surgery. The final result of the surgery can be seen after about 1 year, when the tissues have healed and settled.

You often wonder
If it is a so-called closed approach, the postoperative scars are not visible on the nose at all. The incisions are made on the mucosa about 3-4 mm behind the entrance to the nostrils. If it is a so-called open rhinoplasty, a small scar on the nasal septum is visible.
Although bruising does occur on and around the nose after the procedure, clients experience almost no pain during the healing phase.
The reasons are either aesthetic or in terms of nasal function. Cosmetic modifications concern the outer part of the nose, for example, reducing the bump or reducing the tip of the nose. Functional ones relate to improving the functionality of the nose, such as opening the nasal passages for better breathing. However, this type of surgery is usually performed by ENT (ear, nose, throat) specialists.
Sports or more physical activity can be added gradually. Most often you can start 6-8 weeks after surgery.
Bruising usually subsides 10-14 days after the procedure.
The actual surgical healing is completed after approximately 4 weeks. Even if the nose is already nice in shape for the first month after surgery, there may be occasional swelling, redness of the nose (especially with a sudden change in temperature), sometimes even a slight nasal obstruction caused by temporary swelling of the mucous membrane. Healing and scarring of the internal structures of the nose is a long-term affair.
It is necessary to wear the plaster external fixation for 7-10 days after the procedure. It depends on the type of surgery. The skin of the nose and the sensitive skin of the face then needs a few more days to recover.
The tamponade is removed approximately 1-2 days after the procedure. After that, you can breathe freely through your nose. For the first 10 days after the removal of the tamponade, we do not recommend sniffing, but cleaning your nose several times a day with damp cotton buds.
